In 2003, Top Priority Care Services began as an agency that purposed to serve only two clients. From that arrangement, other needs have been observed and humbly served. Through continued growth, this service and profession has afforded us the opportunity to meet consumers and their families who have overcome some of life�s greatest challenges. As Professional Service Providers, we understand that we are called to serve our clientele and their families as guided by the code of ethics; and the integrity of knowing that the service and care of our consumers must be our �Top Priority�.

Top Priority Care Services offers a variety of services to individuals who have developmental disabilities, behavioral problems and mental health issues. The main goal of this agency is to provide one-on-one services to our consumers that will help build skills and manage symptoms, while improving their quality of life.

The North Carolina Department of Health and Human Services Division of Mental Health Services (NCDHHS/DMHS) is the governing body that regulates all mental health agencies in the State. The NCDHHS/DMHS established Person Centered Thinking (PCT) as the framework for which all Community Support Services are established. Therefore, Top Priority Care Services adapts the PCT model to develop therapeutic interventions in order to provide supports and assistance to the populations served.

Treatment plans are developed based on information provided directly by the consumer.  This information is gathered through an in-take, which includes an interview with the consumer that reveals pertinent information used to create the Person Centered Treatment Plan. Top Priority Care Services utilizes the information received from the in-take to help the consumer develop specific goals that will assist the individual in managing symptoms and building skills that provide alternative behaviors and positive outcomes.

Top Priority Care Services offers the following services:

Community Support

Community Support (CS) consists of mental health and substance abuse rehabilitation services and supports necessary to assist the client in achieving and maintaining rehabilitative, sobriety, and recovery goals. This service is offered to adults and children.

Diagnostic /Assessment

A Diagnostic/Assessment (DA) is an intensive clinical and functional face-to-face evaluation of a recipient�s mental health, developmental disability, or substance abuse condition that results in the issuance of a diagnostic/assessment (DA) report with a recommendation regarding services and provides the basis for the development of a Person Centered Plan (PCP). This service is offered to all TPCS consumers.

Developmental Therapy

Licensed clinicians can provide assessment, individual, and/or family therapy. This service can be provided in the home or in and office setting. This service is offered to adults and children.

Intensive In-Home

Intensive In-Home addresses the needs of children and adolescents who are transitioning from out-of-home placements or are at risk of out-of-home placement and need intensive interventions to remain stable in the community. This service is offered to children and adolescents.

Community Alternatives Program (CAP) �Home and Community Supports

The intent of this service is to meet the habilitation and support needs of individuals living in their own home or their family�s home. Home and Community Supports services provide instruction and assistance to enable the individual to acquire and maintain skills that will allow him/her to function with greater independence in the community. This service is offered to adults and children.

CAP-MR/DD

The Community Alternatives Program for Persons with Mental Retardation/Developmental Disabilities (CAP-MR/DD) is a special Medicaid program started in 1983 to serve individuals who would otherwise require special care in an immediate care facility for people with the mental retardation/developmental disabilities (IICF/MR). It allows these individuals the opportunity to be served in the community instead of residing in an institutional or group home setting. This service is offered to adults and children.

CAP � Personal Care Services

Personal Care Services (PCS) included support, supervision and engaging participation with eating, bathing, dressing, personal hygiene and other activities of daily living. Support and engaging consumer participation is non-habilitative and describes the flexibility of activities that may encourage the person to maintain skills gained during active treatment and/or habilitation while also providing supervision for independent activities of the consumer. This service is offered to adults and children.

CAP�Respite

Respite care is a service that provides periodic relief for the family or primary caregiver. This service may be provided in an individual�s home or in an out-of-home setting.
